BENEFITS
Saint Anthony Hospital has created a comprehensive benefits program to meet the needs of our associates and their families.
Medical
Benefit eligible employees have the option of coverage under the Humana HMO, Humana PPO or Humana HDHP (High Deductible Health Plan). All plans provide access to quality care.
New Hires are eligible for medical benefits on the first of the month following one month of employment.
Costs: Based on the Level and Type of coverage elected.
Dental
Two dental plan options for our benefit eligible associates – Humana DHMO and the Humana PPO plan.
Costs: Based on the Level and Type of Coverage.
Life Insurance
Provided for eligible associates at 1x their current annual salary at no cost. Associates have the option to purchase additional coverage equal to 5x their annual salary.
Long-Term Disability (LTD)
Long Term Disability coverage of 50% of salary is provided at no cost to the associate. Associates can elect 70 % Long-Term Disability coverage for an additional cost.
Legal Plan
The opportunity to purchase access to legal services from the Hyatt Legal Group.
Vision Plan
Vision coverage can be elected and is provided through Humana EyeMed Network.
Flexible Spending Accounts
Health Care Medical Reimbursement Accounts: You may contribute pre-tax dollars in any amount up to $3,000 annually.
Dependent Care Spending Account: You may contribute pre-tax dollars in any amount up to $5,000 annually.
Paid Time Off (PTO)
Provided to cover absences from work including holiday, vacation, sick and personal time off. PTO accrues based on hours worked and length of service. Accruals are pro-rated for those working less than their budgeted hours.
Tuition Assistance
Calendar year maximum of $5,250.00.
Employee Assistance Program (EAP)
Services are confidential and available 24 hours a day at no cost to associates and their family. The program provides professional consultative support services to help associates with work-life issues.

100% match on first 3% contribution of employee eligible earnings
50% match on next 3% contribution of employee eligible earnings
